My name is Kim Allen and I will be at Havelock High School daily from 8:30-2:30.

In order for medication to be taken at school, a Medication Authorization form will need to be filled out by the physicain and signed by the parent.
Students need to stay home if any of these symptoms are present:
severe vomiting or diarrhea
fever greater than 100 degrees
red eyes with drainage
undiagnosed rash
Please inform the nurse of any medical condition that your student has.
Always keep the school informed of any phone number changes so you may be reached in the event of an emergency. |
